Summary

distinguish accepted prompt delivery from observed worker execution and recover no-start attempts
preserve committed, staged, unstaged, and untracked worker progress before destructive lifecycle actions
reconcile restart 404 task ownership, read-only task claims, concurrent preserved refs, and successor wakes

Validation

nix shell nixpkgs#bun --command bun test (985 passed)
nix shell nixpkgs#bun --command bun run lint
nix shell nixpkgs#bun --command bun run typecheck
nix shell nixpkgs#bun --command bun run build
independent QA re-review found no actionable defects
